Halo: Reach keeps freezing...?

My Halo:Reach keeps saying the disc is unreadable after games whenever I try to gain xp or credits from that game, or at the title screen after "Bungie" comes up and as you are flying into the asteroids. I took the game back and exchanged it and this brand new disc does the same thing. Do I need a new xbox or what? What's going on?
